Terapi Budaya: Putus Pengobatan pada Skizofrenia Paranoid

Skizofrenia merupakan gangguan jiwa kronis dengan gejala positif, negatif, dan kognitif, dengan tipe paranoid paling sering dijumpai. Kekambuhan dialami sekitar 80% pasien, terutama akibat penghentian obat. Dilaporkan kasus pria 25 tahun dengan riwayat skizofrenia paranoid yang mengalami kekambuhan setelah 1 tahun putus obat. Gejala berupa halusinasi auditorik-visual, waham kejar dan pengendalian, perubahan perilaku, serta peniruan tokoh. Diagnosis ditegakkan sebagai F20.0 Skizofrenia Paranoid dengan GAF 51–60. Terapi diberikan risperidone 3 mg dua kali sehari serta edukasi kepatuhan. Diskusi menekankan bahwa kekambuhan umumnya dipicu putus obat, sehingga kepatuhan merupakan faktor kunci dalam pencegahan relaps. Optimalisasi terapi memerlukan kombinasi antipsikotik, intervensi psikososial, dan dukungan keluarga. Kasus ini menegaskan pentingnya terapi jangka panjang dan edukasi berkelanjutan untuk mempertahankan perbaikan gejala serta kualitas hidup pasien. Selain itu, terapi memerlukan keterlibatan keluarga, masyarakat, dan lingkungan budaya.
